The situation
A senior care worker who has been reliable for four years has made several recording errors over the last two weeks: two missed entries on a MAR chart signature line, and a daily note written up for the wrong person.
Yesterday, a relative told you she was 'quite short' with them on the phone, which is out of character.
Nobody has raised a concern about the care she gives, and she has not mentioned any problem to you.
